I have searched but have found no clues as to why my conversion is failing.

Anyone have any ideas as to why this conversion is failing?

You need to open a ticket and attach the epub file.

Before opening the ticket, run it through FlightCrew and see if maybe the ePub is not corrupted. I've seen a few where I've had to rebuild them to be able to strip the DRM because either some file in the ePub was invalid or there was a problem with the zip container.
